Request an Appointment
To ensure you receive the right care as efficiently as possible, our online triage system, Patient Triage, remains in place for all appointment and consultation requests. Please submit your requests via Patient Triage instead of calling the practice.

Choose the type of request – Medical or Admin.

Complete the form with as much detail as possible.

Submit your request, ensuring your contact details are up to date.
Your request will be reviewed by a GP, who will determine the best next steps. This may include:

Booking an appointment with a GP or another clinician.

Providing advice or treatment remotely, where appropriate.

Directing you to a more suitable service, such as a pharmacy or physiotherapist.

When to Submit a Request
The triage system is open Monday – Friday, 8am to 1pm.
After 1pm, please contact the practice on 020 8684 5353 to speak to a member of the team who will be able to assist you.
Urgent Medical Issues & Emergencies
- For serious medical emergencies (e.g., severe chest pain, heavy bleeding, collapse, unconsciousness), call 999 immediately.
- For urgent but non-life-threatening issues outside practice hours, contact NHS 111.
Response Times

Urgent requests – assessed and responded to on the same day.

Non-urgent requests – we aim to respond as soon as possible, usually within the same day, but please allow up to 48 working hours.
If your condition changes while waiting for a response, please contact us or seek urgent medical attention.
Cancel an appointment
If you wish to cancel your appointment with short notice, please use the NHS app
If you are unable to use the NHS app, you can call the practice and we will cancel the appointment for you
